
The Nurse Navigator is a liaison with our surgeons, the surgeon’s office, the patient's primary physician and specialty service office practices, Surgical center nurses, Anesthesia, and the whole interdisciplinary team. Reviews for accuracy and ensures correct entry of Surgical postings and appointments in the EMR system. The RN in this role will interview and assess patients, review medications, and ensure all necessary paperwork and diagnostic information is complete, including possible medical or cardiac clearance, sleep studies, lab work, EKG, and any radiologic studies. Maintaining the role of patient advocate. The Nurse Navigator makes sure to interview (either in person, virtual or telephone) all patients who are going to have general or MAC forms of anesthesia. This role will provide assessment, care planning, education, coordination and consultation support to patients, families/caregivers, and clinical staff as the client transitions across the continuum of care.

Some candidates will not be eligible for the upper end of the salary range.
