Job Description

The Practice Success Manager serves as the primary practice-facing relationship owner for an assigned portfolio and market of individual physician practices. They are responsible for both identifying and securing new practice engagements and sustaining performance and satisfaction for practices already receiving Guidehealth’s services. This role represents Guidehealth's operational and clinical infrastructure directly to physicians and practice staff through an ongoing working relationship.

The Practice Success Manager owns the full arc of the individual practice relationship: introducing Guidehealth's services and securing buy-in from practice leadership, transitioning engaged practices into implementation, and maintaining ongoing engagement through monthly performance reviews and coaching. This individual serves as the practice-facing liaison to Guidehealth's clinical pillars (CDI, CM, Quality), represents practice needs internally, and partners directly with practices to resolve service delivery barriers.

WHAT YOU’LL BE DOING

Practice Identification & Pre-Implementation Engagement

  • Identifying and evaluating individual practices within assigned networks for fit with Guidehealth's value-based care services, working within network and health-system relationships established by the Director
  • Conducting outreach and initial meetings with individual practice leadership to introduce Guidehealth's services and value proposition
  • Building the case for engagement and addressing practice-level objections to secure agreement to move into implementation
  • Moving engaged practices into implementation, coordinating with the Practice Operations Manager on kickoff and onboarding

Ongoing Practice Relationship & Retention

  • Serving as the day-to-day point of contact for an assigned portfolio of practices, maintaining consistent communication and touchpoint cadence
  • Building and maintaining trust-based working relationships with practice administrators and frontline clinical staff
  • Supporting long-term retention and satisfaction through consistent, high-quality engagement throughout the performance year
  • Representing practice feedback, needs, and risks internally, routing them to the appropriate team or escalating to the Director as needed
  • Documenting all practice meetings, key email correspondence, and key interactions in HubSpot CRM to maintain an accurate, current practice-level relationship record
  • Scheduling and coordinating practice-facing meetings, including performance reviews, escalation discussions, and ad-hoc check-ins
  • Exercising independent judgment in determining the appropriate meeting cadence and format (in-person versus virtual) for each practice based on relationship stage, performance trajectory, and practice preference, rather than applying a fixed cadence across the portfolio

Performance Review & Coaching Support

  • Providing monthly performance reviews to assigned practices and facilitating the accompanying conversation with practice leadership
  • Translating performance data and benchmarks into clear, actionable next steps for practice teams
  • Supporting practices in developing and tracking improvement plans tied to performance targets, in partnership with the Director
  • Identifying practices at risk of underperformance and escalating with supporting context to the Director

Practice-Facing Coordination

  • Serving as the practice-facing liaison to Guidehealth's clinical pillars (CDI, CM, Quality), communicating updates and workflow changes to practices
  • Serving as the primary internal communication channel between practices and Guidehealth's clinical pillars, relaying practice-level feedback, updates, and operational conditions back to CDI, Care Management, and Quality leads
  • Representing Guidehealth's services and expectations to practice stakeholders during implementation kickoff and onboarding
  • Correcting internal barriers or delays impacting practice service delivery directly with the practice as the first point of resolution
  • Collaborating with the network's Client Relationship Manager (network-side counterpart, external to Guidehealth) when a practice is unresponsive or additional support is needed to resolve outstanding barriers

Network & Client-Level Support

  • Supporting the Director of Practice Success in the development and delivery of network- or client-level presentations and business reviews, contributing portfolio-level performance narrative and practice-level context
  • Keeping the Director informed of network-level trends, risks, and practice sentiment across the assigned portfolio that may affect the broader client relationship

Qualifications

WHAT YOU'LL NEED TO HAVE

  • Ability to travel locally up to 50%.
  • Minimum 3 years of experience in a practice-facing, account management, or business development role within healthcare, value-based care, or a related field.
  • Experience working within an ACO, MSSP, or value-based care model (delegated services, managed care, or population health environments)
  • Demonstrated ability to build trust-based relationships and secure buy-in from physicians and practice leadership
  • Proven ability to facilitate productive, sometimes difficult, performance conversations with practice leadership
  • Strong understanding of individual primary care physician practice workflows, day-to-day operations, and front-office/clinical environments
  • Ability to manage multiple practice relationships simultaneously with high attention to follow-through
  • Strong written and verbal communication skills, with the ability to translate performance data into a clear narrative for practice audiences
  • Strong time and task management skills, including the judgment to calibrate engagement intensity and format across a portfolio of practices with differing needs, rather than treating all practice touchpoints as interchangeable
  • Ability to work independently with strong ownership and accountability for practice-level outcomes
  • Based in or willing to relocate to the Atlanta, GA area, with ability to travel up to 50% of the time within Georgia for in-person practice engagement

WOULD LOVE FOR YOU TO HAVE

  • 4–5 years of experience, or experience specifically in practice acquisition, physician outreach, or provider network development
  • Clinical background (e.g., Medical Assistant, Care Manager, RN, LPN) or other relevant clinical experience
  • Experience in a manager or operations role within a primary care practice
  • Health coach certification or motivational interviewing skills
  • Experience with CRM platforms (HubSpot preferred)
  • Familiarity with quality metrics, HCC coding, or care management integration
  • Existing familiarity with the Atlanta-area independent physician practice landscape

Additional Information

The salary range for this role is between $85,000 and $95,000 per year

All full-time employees of Guidehealth who work 30 hours per week or more are eligible for our comprehensive benefits package. Temporary employees and contractors are not eligible for benefits.

COMPENSATION:

The listed compensation range listed is paid bi-weekly per our standard payroll practices. Final base pay decisions are dependent upon a variety of factors which may include, but are not limited to: skill set, years of relevant experience, education, location, and licensure/certifications.

REMOTE WORK TECHNICAL REQUIREMENTS

Guidehealth is a fully remote company. We provide new employees with the necessary equipment to function in their role at no charge to the employee. Employees provide their own internet connection, capable of conducting video calls on camera and connecting to various internal and external systems.
