Job Description

Join a Mission-Driven Organization Transforming Senior Care

At At Home Harmony, we are redefining how seniors experience healthcare—bringing high-quality, compassionate medical and personal services directly into the home. Based in Richmond and expanding rapidly across Virginia and beyond, our integrated care model blends home-based primary care, long-term care pharmacy, advanced dementia support, and in-home personal care.

Our mission is simple but powerful: help medically complex seniors remain safe, comfortable, and thriving where they want to be—at home.

The Opportunity

We are seeking a dynamic, strategic Practice Administrator to lead operations for a highly complex, fast-growing home-based primary care practice in Richmond, VA.

In this role, you will oversee a large, multidisciplinary team supporting:

  • 15+ providers
  • 20+ nurses
  • Lab Technicians, Medical Assistants, Community Health Workers, and more

This is a high-impact leadership position for an experienced operator who thrives in complex, decentralized healthcare environments and understands how to align clinical excellence with operational and financial performance—especially within value-based care models and ACOs.

You will report directly to the Senior Vice President of Operations and collaborate closely with leadership across finance, technology, and marketing—allowing you to stay focused on driving operational excellence and growth.

What You'll Do

Operational Leadership

  • Lead day-to-day operations of a multi-disciplinary, field-based care model
  • Drive seamless coordination across clinical and support teams
  • Design and implement scalable workflows to support rapid growth
  • Monitor and optimize KPIs related to access, quality, patient satisfaction, and cost

Team Leadership & Culture

  • Inspire, coach, and develop clinical and administrative leaders
  • Build a collaborative, mission-driven culture focused on exceptional patient care
  • Partner with clinical leadership to optimize staffing and workforce utilization

Value-Based Care & ACO Performance

  • Align operations with value-based care strategies and ACO initiatives
  • Drive performance across quality metrics, utilization, and cost of care
  • Collaborate cross-functionally to succeed in risk-based contracts

Clinical Operations Support

  • Partner with medical and nursing leadership to ensure high standards of care
  • Enhance care models for medically complex, homebound populations
  • Ensure regulatory compliance and adherence to best practices

Technology & Systems

  • Optimize use of Athena EMR (preferred)
  • Improve workflows, documentation, and reporting through technology
  • Strengthen data integrity to support quality and financial outcomes

Financial Management

  • Oversee resource allocation and cost controls
  • Partner with finance on forecasting and performance improvement
  • Balance operational efficiency with exceptional patient care

What You Bring

  • Bachelor's degree required; Master's (MHA, MBA, or related) preferred
  • 8–10+ years of progressive healthcare operations leadership
  • Proven success managing complex care delivery organizations
  • Deep experience with ACOs and value-based care models
  • Strong understanding of clinical workflows and healthcare regulations
  • Experience with Athena EMR preferred
  • Demonstrated ability to lead large, multidisciplinary teams

Key Strengths for Success

  • Strategic thinker with strong execution skills
  • Inspiring leader and team builder
  • Data-driven decision maker
  • Financial and operational acumen
  • Change management and process improvement expertise
  • Excellent communicator and collaborator

Why This Role Matters

This is more than an operations role—it's a chance to shape the future of senior healthcare.

You'll lead a model that:

  • Serves high-risk, medically complex patients
  • Improves outcomes while reducing cost of care
  • Allows patients to age safely and comfortably at home

If you're driven by purpose and ready to lead at scale, this is your opportunity to make a lasting impact on patients, families, and the healthcare system itself.
