Schedule: Part-time, days (20 hours/week). No holidays, no weekends.

What You’ll Do

Assist Population Healthcare Coordinators and leadership in implementation of Population Healthcare Programs (i.e. Anthem, Humana, UHC, Aetna, Optum) with focus on direct patient engagement, chart review and data submission, communication with physician office staff in coordinating care management. Responsible for assisting in implementing selected programs and providing continued feedback of programs to Population Healthcare and Business Development leadership. Responsible for maintaining reporting structure of all programs to ensure compliance with Freeman policies and procedures. Responsible for assisting in building direct relationships, communicating with patients, Primary Care Physicians and office staff to ensure successful outcomes in utilization and effective patient care plans. Responsible for completing daily discharge follow-up calls and assisting with the Freeman Physician Finder Line.

Requirements

Knowledge obtained through work experience that would be equal to knowledge obtained by formal education.
2 years preferred experience working in a medical setting.
Current Certification as a Certified Medical Assistant or Registered Medical Assistant.
Minimum 3 years clinical experience.
AHA BLS for Healthcare Providers within 60 days.
