Salary Range: $22.00 - $25.00 Hourly, People Incorporated Mental Health Services is seeking Population health Care Coordinators to join our Outpatient Mental Health teams. The Population Health Care Coordinator (PHCC) is responsible for advancing whole-person, population-based health (physical and mental health) management within the clinic. This role integrates care coordination for clients with co-morbid behavioral, physical, and mental health conditions, focusing on long-term engagement, health promotion, and seamless transitions across services.
The PHCC collaborates with interdisciplinary teams to identify health (physical and mental health) needs, implement targeted interventions, and support clients in achieving individualized goals. Key responsibilities include client engagement, health (physical and mental health education, care coordination, data-driven population health initiatives, and supporting organizational efforts to improve health outcomes for diverse populations.
Schedule: Full-Time. Monday-Friday, Regular clinic hours, some clinics stays open until 6pm.

Hiring Range: $22.00 - $25.00/ hourly, plus $1,000 hiring bonus for external candidates
