Population Health & ACO Operations Coordinator-(26003430)

The Population Health & ACO Operations Coordinator will support the operational management and execution of Population Health and Accountable Care Organization (ACO) programs and initiatives.This role partners closely with clinical and administrative leaders to implement workflows, support oversight of program performance, support compliance and quality improvement, and ensure efficient delivery of Population Health and ACO activities. The coordinator collaborates across departments, and support relationships with payers and other external partners to drive excellence in care delivery and patient experience within value-based programs.

Position Responsibilities:

ACO and Value-Based Contract Operations

  • Assist with the build and management of operational processes and infrastructure required to support Medicare Shared Savings Program (MSSP) ACO and other value-based contracts.

  • Support relationships with external value-based partners, such as the ACO; support action planning, performance tracking, and stakeholder engagement.

  • Monitor and ensure compliance with CMS and payer-specific ACO requirements.

  • Support onboarding and implementation activities for new value-based agreements, including contract deliverables, data transfers, and network coordination.

Population Health Data and Analytics

  • Support implementation and maintenance of Population Health components of Healthy Planet

  • Collaborate with IT, clinical informatics, and operational leaders to expand Healthy Planet capabilities and optimize care workflows.

Operational and Program Management

  • Monitor financial performance of assigned projects or programs.

  • Identify process improvement opportunities and implement changes to improve performance, quality, and patient outcomes.

  • Provide operational support for core functions such as care gap closure, chronic condition management, and provider engagement strategies.

  • Support internship program operations

Planning, Communication, and Collaboration

  • Support strategic operational plans, support progress monitoring, and communicate data and progress to Population Health leadership

  • Support coordination with departments across UI Health Care (e.g., IT, Finance, Compliance, Clinical Leadership) for cross-functional initiatives.

Compliance, Quality, and Reporting

  • Maintain awareness of and alignment with federal, state, and payer-specific regulatory requirements.

  • Support quality improvement initiatives and action planning tied to clinical performance, patient satisfaction, and contract compliance.

  • Review and translate dashboards and reports to support presentations for leadership and stakeholder groups

Qualifications

Required Qualifications:

  • Bachelor’s degree in Health Administration, Public Health, Business, Nursing, or related field or the equivalent combination of education and experience.

  • 3+ years of progressively responsible experience in healthcare operations, population health, value-based care, or program management.

  • Demonstrated experience in project management and cross-functional collaboration.

  • Strong communication and interpersonal skills.

  • Proficiency in using data to inform decisions and monitor performance.

  • Demonstrated experience working effectively in a welcoming and respectful workplaceenvironment.

Desired Qualifications:

  • Master’s degree in Public Health, Health Administration, or Business Administration.

  • Experience with accountable care organizations (ACOs), Medicare Shared Savings Program (MSSP), and/or commercial value-based contracts.

  • Knowledge of clinical operations, health equity strategies, and regulatory compliance in healthcare settings.

  • Familiarity with CMS ACO compliance, quality reporting, and data infrastructure.

  • Familiarity with Lean, Six Sigma, or other quality/process improvement methodologies.
