Department: Care Center

Shift/schedule: Full Time (40 hrs/wk)

GENERAL SUMMARY

Works under the supervision of the Population Health LPN Nurse Manager. The Patient Care Coordinator’s primary job function is to perform clerical duties associated with answering the phone, scheduling of patients, tracking patient visits, reviewing documentation for appointment agendas, and assisting patients with social determinants of health when necessary.

Education:

·High School Diploma or successful completion of an equivalent High School Exam Required

·Computer courses preferred

Licensure:

·None

Experience:

·Working knowledge of Microsoft Word, PowerPoint and Excel required.

·Previous clerical, computer, and/or similar office experience preferred

JOB SPECIFIC DUTIES AND PERFORMANCE EXPECTATIONS

The following is a summary of the major job duties of this job. Other duties may be performed, both major and minor, which are not mentioned below. Specific activities may change from time to time.

1.Demonstrates a thorough knowledge of all computer fields (Meditech registration fields, scheduling, etc and payer portals).

2. Answers multi-line phones and schedules appointments with a positive and helpful attitude.

3. Schedules referrals and outpatient testing/procedures.

4. Assists in the coordination of care for the patient in collaboration with other members of the health care team to meet the health care needs of the patient.

5. Utilizes computers for data, information retrieval, and printing reports.

6. Participates in value-based care activities with documentation review and completing data collection for various payers.

7. Assists in preparation, organization, and form completion as needed for Quality & Value-Based Care programs.

8. Assists and provides support in collecting, organizing, and summarizing data.

9. Participates in community events as needed.

10. Retrieves mail and faxes and distributes those accordingly.

11. Performs other duties as assigned.
