Richmond, VA | Flexible Scheduling

About the Role

Legacy Care is seeking a compassionate Psychiatric Mental Health Nurse Practitioner (PMHNP) to provide talk therapy and psychiatric medication management to residents in post-acute, long-term care, and assisted living settings in Richmond, VA.

This role combines psychiatric assessment, therapeutic conversations, and medication management to address each patient’s emotional and behavioral health needs. Providing talk therapy is a core responsibility of this position, with interventions tailored to each patient’s goals, cognitive abilities, and clinical needs.

You will work alongside nursing staff, primary care clinicians, and interdisciplinary teams to support patients experiencing depression, anxiety, grief, adjustment difficulties, and other mental health concerns.

Schedule

This part-time opportunity offers a flexible schedule coordinated around facility coverage needs and mutual agreement. Care is provided through scheduled daytime, on-site visits at assigned facilities, with outpatient consultations as needed.

Core Responsibilities

  • Conduct comprehensive psychiatric assessments, mental health screenings, and risk evaluations.
  • Provide individual talk therapy when needed, including supportive psychotherapy and other evidence-based therapeutic interventions within your training and scope of practice.
  • Help patients develop coping skills and navigate grief, illness, changes in independence, and adjustment to facility living.
  • Develop individualized treatment plans that incorporate psychotherapy, medication management, and non-pharmacologic approaches as clinically appropriate.
  • Prescribe and manage psychiatric medications within your scope of practice, monitoring treatment response, side effects, and potential interactions.
  • Assess ongoing progress and adjust therapeutic approaches and medication plans to support patient goals.
  • Provide crisis assessment, de-escalation, and behavioral support in collaboration with facility staff.
  • Coordinate referrals for additional therapy, specialty psychiatric care, or community resources when needed.
  • Collaborate with nursing teams, primary care providers, social workers, and other clinicians on care planning and transitions.
  • Educate patients, families, and staff about mental health concerns, treatment options, and supportive care strategies.
  • Complete timely, accurate EMR documentation of assessments, psychotherapy interventions, medication management, and follow-up plans.
  • Participate in care conferences, case reviews, and quality improvement activities as appropriate.

Qualifications

  • Master’s or doctoral degree from an accredited nurse practitioner program with a psychiatric mental health specialty.
  • Current PMHNP board certification.
  • Active, unrestricted Virginia licensure appropriate to the role and current DEA registration as required for prescribing.
  • Current BLS certification and any additional credentials required by assigned facilities.
  • Ability and willingness to provide talk therapy alongside medication management, with experience in supportive psychotherapy or other evidence-based approaches preferred.
  • Experience in geriatrics, post-acute care, long-term care, or comparable behavioral health settings preferred.
  • Strong clinical judgment, therapeutic communication, and documentation skills.
  • Ability to practice autonomously while collaborating with an interdisciplinary team.
  • A patient-centered, culturally responsive, and trauma-informed approach to care.
