JOB SUMMARY

HealthRIGHT 360, a nonprofit organization and a family of programs, is committed to providing accessible and comprehensive healthcare services to vulnerable populations. The Specialized Treatment for Optimized Programming (STOP) Area Six Program connects California Department of Corrections and Rehabilitation inmates and parolees to comprehensive, evidence-based programming and services during their transition into the community, with priority given to those participants who are within their first year of release and who have been assessed as a moderate to high risk to reoffend. Area Six includes San Diego, Orange, and Imperial counties. STOP subcontracts with detoxification, licensed residential treatment programs, outpatient programs, professional services, and reentry and recovery housing throughout the program area to assist participants with reentry and recovery resources.

The Placement Coordinator manages the intake, assessment, and placement of parolees into appropriate community-based programs. They coordinate with CDCR, parole agents, and a network of subcontracted providers to match clients to the right level of care, arrange transportation, and ensure seamless transitions. The role involves maintaining accurate client records, data entry, reporting, and compliance with contractual and confidentiality requirements. Coordinators also communicate with community agencies, conduct presentations, assist in training new staff, and serve as an incident reporting designee as needed. The primary focus is on client placement, with additional administrative, data management, and collaborative responsibilities.

KEY RESPONSIBILITIES

Client Intake and Referral Management

  • Receive referrals for parolees from in-prison programs and DAPO Parole Units.
  • Proactively link clients to Community-Based Providers (CBPs) based on treatment needs/level of care.
  • Screen client profiles for possible barriers to treatment delivery.
  • Assess, schedule, and coordinate placements of project referrals into appropriate community placements.
  • Receive transitional plans from CDCR and County institutions.

Placement Coordination

  • Match inmates/parolees to the appropriate level of care within a network of sub-contracted CBPs.
  • Submit service authorizations to CBPs.
  • Coordinate transportation with Transportation Coordinator.
  • Notify AORs (Authorized Organizational Representatives) of client placement.
  • Maintain and update bed counts and reporting spreadsheets.
  • Track release dates and coordinate client releases with transportation services.

Collaboration

  • Conduct daily communication with Parole Agents.
  • Communicate with CDCR personnel, community-based subcontractor staff, institutional SAP staff, Parole, Community Transition Program (CTP), County Jails, STOP staff, and other internal/external stakeholders.
  • Conduct presentations to community agencies to increase referral pool.
  • Provide technical assistance and presentations for CBP staff.
  • Identify community capacity needs.

Data Management and Documentation

  • Enter placement notes into data systems (including ARMS).
  • Maintain accurate records and documentation in accordance with HealthRIGHT 360 guidelines.
  • Create, open, update, close, and archive client files.
  • Ensure client files meet contractual and compliance requirements.
  • Conduct file compliance checks and reconciliation.
  • Compile, sort, and verify accuracy of data to be entered.
  • Contact preparers of source documents to resolve inconsistencies or missing data.
  • Generate reports and respond to inquiries regarding entered data.
  • Submit reports to QA/Fidelity Supervisor, Case Manager Supervisor, and/or Program Director.
  • Maintain and update spreadsheets and other tracking systems.

Administration and Other Duties

  • Perform general clerical tasks (typing, answering phones, filing, processing forms).
  • Maintain an effective and efficient records management system.
  • Ensure confidentiality requirements are met.
  • Attend required trainings and meetings.
  • Assist in training new hires.
  • Serve as an Incident Reporting designee as needed.
  • Complete assigned training in a timely manner.

And perform other duties as assigned.

QUALIFICATIONS

Credentials and Experience

  • High school diploma or equivalent.
  • Minimum 2 years’ experience working with the criminal justice population.
  • SUD treatment/recovery program experience preferred.
  • Valid First Aid and CPR certification or ability to obtain within 30 days of hire.
  • Valid California Driver’s License and access to registered and insured transportation.

  • Presentation & Training Skills. Conducts presentations for community agencies and assists in staff training.
  • Referral & Placement Coordination. Matches clients to suitable programs and coordinates transportation and services.
  • Technical Proficiency. Utilizes data systems, spreadsheets, and electronic records for accurate tracking.
  • Transportation Coordination. Plans and arranges client transportation to placements as needed.
  • Travel Readiness. Willingness and ability for outreach, case management, and assessments, including overnight stays as needed.
  • Bilingual preferred.
