Shifts: Monday - Friday 9 am - 5 pm with occasional evenings.

As an integral member of the collaborative, interdisciplinary Family Health Team, the Physiotherapist provides physiotherapy services to patients of the Toronto Western Family Health Team – both Bathurst and Garrison Creek Sites. The Physiotherapist enhances quality of life through diagnosis of diseases or disorders associated with physical dysfunction, injury or pain and the treatment, rehabilitation and prevention or relief of physical dysfunction, injury or pain to develop, maintain, rehabilitate or augment function and promote mobility. The Physiotherapist offers assessment of a wide variety of presentations seen in the primary care setting and works within the interprofessional team in a short term treatment model focused on assessment and education. Principal responsibilities include; clinical care, education of patients families, students and staff, program development, and participation in research and quality improvement initiatives. Working closely with patients, families and the interprofessional team, the Physiotherapist delivers patient centred care and contributes to collaborative case management within a dynamic healthcare environment.

Duties

  • Performs assessment/consultation of physical function.
  • In collaboration with the patient and other health care professionals, develops and implements individualized treatment plans with the goal of improving physical functioning, alleviating pain and preventing physical dysfunction in patients.
  • Examines and re- evaluates the outcomes of treatment.
  • Coordinates and/or participates in the education of students/learners, hospital staff and the community as well as maintaining excellence in the provision of physiotherapy services through ongoing professional development.
  • May conduct practice-based research and participate in physiotherapy and interprofessional research or quality improvement activities
  • Performs quality improvement and administrative duties to ensure the efficient and effective operation of the Family Health Team.

Qualifications

  • Successful completion of a Master’s Degree in Science (Physiotherapy) or recognized equivalent required.
  • Registration in good standing with the College of Physiotherapists of Ontario required.
  • Membership in the Canadian Physiotherapy Association preferred.
  • Membership in the Ontario Physiotherapy Association preferred.
  • Minimum one (1) year of related clinical experience required.
  • Experience in primary care or community care settings preferred.
  • Experience in the assessment and treatment of neurological, cardiopulmonary, orthopedic, and chronic disease conditions required.
  • Experience in health promotion, prevention, and chronic disease management preferred.
  • Client service–oriented, with the ability to work effectively within a diverse environment and value differing perspectives and backgrounds.
  • Excellent problem-solving skills required.
  • Ability to exercise sound clinical judgment and function independently in a self-directed manner required.
  • Ability to function effectively as an integral member of an interprofessional team.
  • Demonstrated ability to offer solutions to problems and seek guidance from supervising staff when appropriate.
  • Experience in program planning, delivery, and evaluation preferred.
  • Experience with Telus PSS EMR considered an asset.
  • Demonstrated ability to perform duties in a professional and courteous manner and produce high-quality work while meeting deadlines in accordance with UHN standards.
  • Demonstrated initiative and sound judgment, with the ability to manage competing priorities effectively.
  • Compliance with confidentiality requirements required.
  • Effective organizational, interpersonal, and communication skills required.
  • Knowledge of applicable legislation, the College of Physiotherapists of Ontario standards, and UHN and/or departmental policies required.
  • Compliance with the Regulated Health Professions Act (2011) required.
  • Compliance with the Code of Ethics and Rules of Conduct of the College of Physiotherapists of Ontario required.
  • Demonstrated competence in departmental and hospital safety, emergency, and infection prevention and control procedures required.
  • Demonstrated ability to recognize and practice within the scope and limitations of the physiotherapist role required.
  • Computer proficiency required.
  • Ability to work in a fast-paced or stressful environment while maintaining professionalism and appropriate boundaries.
