The Nephrologist is responsible for providing expert consultation and continuing care for patients with renal-relatedconditions, including chronic kidney disease, acute kidney injury, and hypertension, while offering leadership to thecomprehensive care team managing complex medical issues. The physician works collaboratively within amultidisciplinary team to deliver high-quality, patient-centered care that aligns with Central Health’s mission toimprove the health and well-being of the communities we serve. The role may also include supervising advancedpractice providers and serving as an attending physician for learners at various levels. Preferred candidates willhave experience in FQHC or community health settings, demonstrate cultural competence in caring for diversepopulations, and excel in preventive care and patient education. Familiarity with value-based care models, EHRsystems, and telehealth is highly desirable, as is a commitment to addressing social determinants of health andreducing health disparities.Central Health's mandate is to serve as the safety net for individuals in the county who need healthcareservices. MD or DO from an accredited medical school
Completion of a Residency in Internal Medicine
Residency in InternalMedicine and Accredited Fellowship in Nephrology with Certification from the American Board of Medical Specialties.
