About this role:
As a Physician in LifestyleMedicine,you will work with a team of healthcare professionals committed to providing quality care andanexcellentpatient experienceIn thisrole,you willprovide clinical services topatientsforpreventing, treating, and managing chronic diseases andimproving quality of life
This position includes a $15,000sign-on bonus,and up to $20,000in relocationassistance
You will practice at our Intermountain Health Live Well Center - Park City, UT
Experienced Lifestyle Medicine Physicians and New Graduates with Wellness Certification will be considered

Whatyou’llbring:
MD or DO degreefrom a fully accredited medical or osteopathic school of medicine
Residency traininginthedesignateddiscipline
Board certification or eligibility inthe designated discipline
Lifestyle / Wellness Certification
Active, unobstructedUtahmedicallicense, or the ability to obtain one
Full prescriptive authority and an active DEA, or the ability to obtain one
Ability to successfully complete Intermountain Health’s credentialing process
