We are seeking a compassionate and experienced Primary Care Physician Assistant or Nurse Practitioner to join our dynamic team at Foundation Medical Partners / Southern NH Health. In this role, you will play a vital role in providing comprehensive care to a diverse patient population in Nashua, NH.

• 40 Hours – 50% direct patient care work and 50% coordinating and communicating patient care

Qualifications

• Certified Physician Assistant or FNP
• 2+ years of Primary Care experience
• Strong clinical skills, including assessment, diagnosis, and treatment planning.
• Excellent communication, collaboration, and interpersonal skills.
• Proficiency with EPIC EMR preferred
• Ability to work independently and as part of a team.
