
ChristianaCareis currently seeking a part-time hybridadvanced practice provider (PA/NP)for theChristianaCareED callback team. In this role you will work collaborativelywithin the service line and with other departments to ensure timely follow-up of ED results.The APP will utilize established protocols to determine the need for further evaluation, testing,ortreatmentand will communicate this information directly topatient andorother healthcare providers.
This is a part-timedayshiftposition covering 40 hours biweeklywith flexibility to allow forsome of these hoursto bedone virtually.The selected candidate will work alongside other callback providers to provide coverage 7 days a weekincluding holidays. This role oversees results for CCHS’ Delaware and Maryland campuses.
PrincipalDutiesand Responsibilities
Reviewall results (lab and radiology)forpatientswho are discharged fromaCCHS Emergency Department
Contact patients toinform ofpertinentpost-dischargeresultsper callback guidelinesand perform necessary follow-up actions which include,but are not limited to,prescribing medications, coordinating outpatient follow-up care, and patient education
Receive and manage incoming calls from patients and pharmaciststo assist withvisit relatedquestions andprescriptionquestions/medication adjustments. Return voicemail messagesreceivedduringoff hours
Coordinatetransparency for blood and body fluid exposures involving non-CCHS healthcare professionalsseen in the Emergency Department
Initiatereferralsto appropriate resources and services, promoting acollaborativeapproach to patient care
Communicatewith other departments/providers as needed (ie: radiology, lab, inpatient teams, specialists,or family physicians)
Document in EHRto maintain complete and accurate record of follow-up care
Work with the ED Administration residents and ED PA Fellows to educate them on the workingsof the callback role
Perform additional callback related duties as they arise

Annual Compensation Range $125,361.00 - $161,327.00This pay rate/range represents ChristianaCare’s good faith and reasonable estimate of compensation at the time of posting. The actual salary within this range offered to a successful candidate will depend on individual factors including without limitation skills, relevant experience, and qualifications as they relate to specific job requirements.
