Job Description

Care that goes beyond the clinic

  • Clover Health is changing how care is delivered for people with complex health needs.
  • Our In-Home Care team brings compassionate, high-quality primary care directly into members’ homes.
  • Supported by care teams and Clover Assistant, our data-driven platform, you’ll deliver proactive, relationship-based care where it matters most.
  • We’re looking for a Physician Assistant in Atlanta who wants to manage complex care, build real patient relationships, and make a measurable impact.

What you’ll do

  • Deliver longitudinal primary care to Clover members in their homes.
  • Manage a dedicated panel of patients with chronic, complex, and advanced medical needs.
  • Build trusted relationships with members, caregivers, and families.
  • Assess clinical needs, create care plans, and support patients through ongoing care.
  • Partner with physicians, care teams, and community providers to improve outcomes.
  • Use Clover Assistant and clinical data to guide proactive, personalized care.
  • Help prevent avoidable emergency room visits and hospitalizations.
  • Participate in a shared on-call rotation to ensure members have continuous support.

What winning looks like

  • Members feel seen, heard, and cared for.
  • Patients with complex needs receive consistent, proactive care at home.
  • Care plans are clear, personalized, and followed through over time.
  • Clinical issues are caught earlier, before they become crises.
  • Families and caregivers trust you as a steady clinical partner.
  • You help improve quality, outcomes, and the overall member experience.
  • Your panel is managed with confidence, compassion, and clinical ownership.

Why this role is meaningful and worth it

  • You’ll care for patients with complex conditions, social needs, and real-world barriers to care.
  • You’ll work independently in the home while staying connected to a broader care team.
  • You’ll need strong clinical judgment, flexibility, and emotional intelligence.
  • Every home visit is different, and every patient needs something personal.
  • The impact is direct: better care, fewer gaps, stronger relationships, and healthier members.
  • This is not transactional care — it’s relationship-based medicine for people who need it most.

Who you are

  • Certified Physician Assistant, PA-C.
  • Located in or near Atlanta, Georgia.
  • Able to travel locally for in-home visits.
  • At least 1 year of PA experience in a clinical setting.
  • Active DEA registration.
  • Current Basic Life Support certification.
  • Comfortable managing chronic illness and medically complex patients.
  • Confident working with autonomy while collaborating with physicians and care teams.
  • Compassionate, grounded, and able to build trust quickly with patients and families.
  • Organized, responsive, and comfortable using technology to support care delivery.
  • Home care, house call, geriatrics, complex care, or value-based care experience is a plus.

About Clover Health

Clover Health (Nasdaq: CLOV) is a physician enablement company focused on seniors who have historically lacked access to affordable, high-quality healthcare

We aim to provide great care, in a sustainable way, by having a business model built around improving medical outcomes while lowering avoidable costs. We do this while taking a holistic approach to understanding the health needs and social risk factors of those under our care. This strategy is underpinned by our proprietary software platform, the Clover Assistant, which is designed to aggregate patient data from across the health ecosystem to support clinical decision-making by presenting physicians and other providers with real-time, personalized recommendations at the point of care.

Making care more accessible is at the heart of our business, and we believe patients should have the freedom to choose their doctors. We offer affordable Medicare Advantage plans with extensive benefits, provide primary care physicians with the Clover Assistant, and also make comprehensive home-based care available via the Clover Home Care program.

With our corporate headquarters in Nashville, Clover’s workforce is distributed around the U.S. and also includes a team of world-class technologists based in Hong Kong. We manage care for Medicare Advantage members in Alabama, Arizona, Georgia, Mississippi, New Jersey, Pennsylvania, South Carolina, Tennessee, and Texas.

We are hiring software engineers, data scientists, designers and product folks who can help us understand our members’ wellness and steer them clear of any health risks down the road.

If you’re a passionate person and interested in changing healthcare for the better, then Clover Health may just be the place for you.
