Job Location: 341 Magnolia Ave Ste 207 - Corona, CA 92879
Salary Range: $80.00 - $90.00 Hourly
- Patient Assessment & Care:

- - Conduct comprehensive cardiovascular assessments, including health histories and physical exams.

- - Order and interpret diagnostic tests (e.g., EKGs, stress tests, echocardiograms, etc.).

- - Provide education to patients on heart disease prevention, management, and lifestyle changes.

- Diagnosis & Treatment:
-
- - Diagnose and manage common and complex cardiovascular conditions such as hypertension, arrhythmias, heart failure, coronary artery disease, and more.

- - Develop, implement, and adjust individualized care plans in collaboration with the cardiology team.

- - Prescribe medications and monitor their efficacy and side effects.

- - Provide recommendations for non-pharmacologic interventions such as dietary changes, exercise plans, and smoking cessation.

- Patient Management:

- - Monitor chronic conditions, coordinate follow-up visits, and adjust treatment as necessary.

- - Assist in managing acute cardiac events or complications, providing emergency care if needed.

- - Review lab results and adjust treatment plans accordingly.

- Collaboration & Communication:

- - Collaborate with cardiologists, nursing staff, and other healthcare providers to ensure comprehensive, high-quality patient care.

- - Communicate effectively with patients, providing clear instructions and answering questions regarding diagnosis and treatment plans.

- - Participate in case discussions, peer reviews, and continuing education to stay updated with current cardiovascular practices.

- Patient Education:

- - Educate patients and their families on their conditions, treatment options, and preventative care.

- - Discuss lifestyle modifications (e.g., diet, exercise, stress management) to improve cardiac health.

- - Prepare patients for invasive procedures or surgeries (e.g., catheterization, angioplasty, etc.).

- Administrative Duties:

- - Maintain accurate and up-to-date patient records in compliance with clinic policies and healthcare regulations.

- - Participate in patient care meetings, quality assurance activities, and other clinic-related initiatives.

QualificationsEducation:
- Master’s degree in Nursing from an accredited Nurse Practitioner program.
-
Experience:- Minimum of [2-3] years of experience as a Nurse Practitioner, with significant exposure to cardiology or cardiovascular care.
- Experience in managing a variety of heart diseases and cardiac conditions is highly preferred.

- Ability to work independently and as part of a multidisciplinary team.
- Proficient in electronic medical records (EMR) and other relevant healthcare technology.
