About the Practice

Burnt Hills Pediatrics and Internal Medicine, part of Community Care Physicians (CCP), is a well-established primary care practice providing compassionate, evidence-based care for patients of all ages. Our physicians are trained in Pediatrics or are dual board-certified in Internal Medicine and Pediatrics (Med-Peds), offering a unique ability to care for both children and adults within the same practice.

Serving the community since 1998, our team is dedicated to building long-term relationships with patients and families while delivering coordinated, high-quality care. We are recognized as a Patient-Centered Medical Home (NCQA), reflecting our commitment to accessible, team-based, and patient-focused care.

Our collaborative care model includes physicians and advanced practitioners working together to support preventive care, chronic disease management, and lifelong wellness.

We are seeking a full-time Physician to join our collaborative primary care team. This role is ideal for a provider interested in caring for patients across the lifespan, with flexibility for those trained in Med-Peds, Internal Medicine, or Family Medicine.

The ideal candidate is committed to building long-term patient relationships, delivering high-quality outcomes, and working in a supportive, team-based environment.

Key Responsibilities

  • Provide comprehensive primary care services to patients of all ages (pediatric through adult).
  • Diagnose and manage acute and chronic medical conditions.
  • Perform preventive care, wellness visits, and age-appropriate screenings.
  • Deliver continuity of care across life stages, supporting both individual and family health needs.
  • Collaborate with physicians, nurse practitioners, and physician assistants to ensure coordinated care.
  • Utilize electronic health records, telehealth, and patient portal tools to enhance access and efficiency.
  • Participate in quality improvement initiatives and practice development efforts.
  • Educate patients and families on health promotion, disease prevention, and treatment plans.

Qualifications

Qualified candidates should have completion of Doctor of Osteopathy (DO) or Doctor of Medicine (MD), completion of an accredited internship and residency and an active license to practice medicine in NYS. Must be currently board certified by an American Board of Medical Specialties (ABMS) Member Board or an approved American Osteopathic Association (AOA) specialty board. Upcoming new graduates will still be considered in a board eligible status.
