Community Care Physicians – CapitalCare Family Practice Slingerlands
Community Care Physicians (CCP), the largest independent multispecialty medical group in the Capital Region, is seeking a Family Medicine Physician to join CapitalCare Family Practice Slingerlands, a well-established primary care practice providing comprehensive family medicine services to patients of all ages in a welcoming, team-based environment.
As a Physician at CapitalCare Family Practice Slingerlands, you will provide full-spectrum family medicine care alongside experienced physicians and Advanced Practice Providers. The practice emphasizes preventive medicine, chronic disease management, and strong patient relationships, with modern tools such as telemedicine and a secure patient portal to support coordinated care and patient access.
Conduct comprehensive patient evaluations and manage acute and chronic conditions
Diagnose illnesses, develop treatment plans, and prescribe medications as appropriate
Order and interpret diagnostic tests and coordinate specialty referrals as needed
Provide preventive care, annual physicals, and wellness visits across the lifespan
Collaborate closely with Advanced Practitioners, nurses, and support staff to ensure continuity of care
Maintain accurate and timely clinical documentation in accordance with organizational standards
Care across the lifespan – serving pediatric, adult, and geriatric patients in a community-based family medicine setting
Early morning weekday hours, offering convenient access for patients and structured scheduling for providers
Telemedicine visits and secure patient portal, supporting flexible access and ongoing patient communication
Established collaborative care team of physicians and Advanced Practice Providers serving a strong patient panel
Convenient Slingerlands location, serving Albany County and surrounding communities with a stable, long-standing patient base
Competitive benefits and compensation package including a base salary of $275,000 and 100% covered health insurance!
Qualifications
Qualified candidates should have completion of Doctor of Osteopathy (DO) or Doctor of Medicine (MD), completion of an accredited internship and residency and an active license to practice medicine in NYS. Must be currently board certified by an American Board of Medical Specialties (ABMS) Member Board or an approved American Osteopathic Association (AOA) specialty board. Upcoming new graduates will still be considered in a board eligible status.
