Practice medicine that matters. At EBCAP, you'll care for individuals and families across all ages and stages of life in a community health setting built on compassion, integration, and purpose.

This is a full-scope primary care role embedded in EBCAP's integrated care model. You'll work alongside behavioral health clinicians, care coordinators, and specialists to deliver whole-person care to some of Rhode Island's most underserved individuals and families.

WHAT YOU'LL DO

  • Conduct comprehensive patient examinations using appropriate medical instruments and diagnostic equipment
  • Gather, document, and maintain accurate patient medical histories within the electronic medical record system
  • Order, perform, and interpret diagnostic tests, laboratory analyses, and imaging studies to evaluate patient conditions
  • Diagnose and treat a wide range of acute and chronic medical conditions
  • Prescribe medications, administer treatments, and provide preventive care services, including vaccinations
  • Educate patients on health and wellness topics such as nutrition, exercise, hygiene, and disease prevention
  • Provide prenatal and postnatal care services
  • Coordinate referrals to specialists and additional care providers as needed
  • Collaborate closely with nurses, physician assistants, medical assistants, and other allied health professionals
  • Provide clinical supervision and guidance to support high-quality patient care delivery
  • Contribute to a collaborative, integrated care model across the health center
  • Partner with internal programs and services to ensure coordinated patient care
  • Support the goals and initiatives of the Integrated Care Coordination Team
  • Deliver safe, effective, and patient-centered clinical care
  • Maintain accurate records, documentation, and reports related to services provided
  • Participate in professional meetings, trainings, and relevant organizational activities
  • Identify and communicate clinical issues, operational concerns, or opportunities for improvement to the Medical Director
  • Maintain current knowledge, clinical skills, and proficiency across relevant areas of medicine
  • Stay informed on community health concerns, including child abuse prevention, domestic violence awareness, and public health initiatives
  • Perform additional duties and responsibilities as assigned by the Medical Director

WHAT WE'RE LOOKING FOR

  • Degree from an Accredited Doctor of Medicine (M.D.) or Doctor of Osteopathic Medicine (D.O.) program
  • Completion of residency in Family Medicine or equivalent
  • Active, unrestricted Rhode Island medical license
  • Board certification or eligibility in relevant specialty:
    • MDs: American Board of Family Medicine certification
    • DOs: American Osteopathic Board of Family Physicians certification
  • Current DEA registration
  • BLS certification required; ACLS preferred
  • Clinical knowledge spanning neonates through older adults
  • Experience with women's health, pre/post-operative care, and psychiatric recognition
  • Comfort with electronic medical records and AI-assisted documentation tools
  • Cultural competency and sensitivity to socioeconomic factors affecting care
  • Commitment to health equity and serving diverse, underserved populations
  • Multilingual (Spanish / Portuguese) strongly preferred

About East Bay Community Action Program

East Bay Community Action Program (EBCAP) is a private, non-profit 501 (c) (3) corporation. The mission of EBCAP is to provide high-quality, comprehensive and accessible health and human services to assist people to achieve their fullest potential.

In 2016 EBCAP merged with East Bay Center (EBC), a state recognized Community Mental Health Center (CMHC) and Integrated Health Home. Given that the two were recognized at the local, state, and national levels as premier providers, the transformation and merger into a single organization greatly enhanced the level of expertise and expanded the breadth and depth of services that EBCAP could offer to the communities it serves.

With this merger, EBCAP received provisional approval by the state as an Accountable Care Entity, an in-house partnership of medical, dental, behavioral health, and social service providers working together to create a shared culture of collaboration and transformation of the health care system. The ACE will focus on supporting high quality care, enhancing health information technology, and experiencing shared savings through agreements with major Medicaid providers, such as Neighborhood Health Plan of RI.
