Position Summary

The IDD Rehabilitation Manager will be responsible for ensuring clinical supervision of the IDD rehabilitation staff and ensure the delivery of competent, effective and safe rehabilitation services, including, but not limited to physical therapy, occupational therapy, and speech language pathology. The IDD Rehabilitation Manager will function under direction of the Director of Rehabilitation services to train, supervise, instruct and guide therapy personnel to assure the high quality of patient care services.

Responsibilities

  • Directs, coordinates, and evaluates the delivery of Home Health services.
  • Monitors IDD Rehab staff assignments, caseloads, service utilization, and team productivity.
  • Provides direct supervision of field staff, including on-site supervisory visits & chart reviews.
  • Provides oversight into contract rehabilitation services to Extended Home Care and ensures compliance with regulatory requirements and clinical excellence.
  • Competent to apply age-specific criteria as appropriate. Responsible for the assessment and treatment for patients of all ages, including neonates, children, adolescents, and adult patients.
  • Understands the cognitive, physical, emotional and chronological processes for all ages.
  • Appropriately assesses patient eligibility for admission to home care and facilitates discharge planning when appropriate.
  • Ensure all documentation is timely and accurate, including OASIS, plans of care and daily visit documentation.
  • Liaises with intake and scheduling department to coordinate patient referrals, patient care and patient assignments.
  • Assuring the formulation, implementation and updating of an individualized care plan for each patient admitted to the home care.
  • Assures communication among physicians or non-physician providers involved in the plan of care.
  • Participates and provide training and continuing education as necessary.
  • Participates in Rehab Service orientation for IDD clinicians as necessary.
  • Evaluates and ensures competency requirements are met for all clinical staff.
  • Identifies appropriate staffing patterns and needs of assigned staff.
  • Demonstrates the ability to recognize and develop the capabilities of others.
  • Implements policies and procedures for the Rehab services and makes the information available to all personnel.
  • Completes IDD staff performance appraisals and field evaluations within specified time frames.
  • Coordinates patient care and agency activities by assisting with problem solving and prioritizing.
  • Identifies and resolves department’s internal problems with staff; develops alternative solutions when necessary.
  • Provides Director of Rehab services with census, staffing, team productivity and other reports as requested.
  • Responsible for performance evaluations and disciplinary action of all assigned staff.
  • Coordinates appropriate communication and documentation between patients, agencies, medical professionals, and referral sources whenever is needed.
  • Evaluates and suggests revisions to clinical documentation system.
  • Evaluates therapy documentation.
  • Assists with assigned performance improvement data collection.
  • Participates in case conferences and/or Interdisciplinary team conferences as needed
  • Assigns staff to Interdisciplinary team as needed to review care delivery and promote interdisciplinary management of patients admitted to the home care department.
  • Works with other members of the Interdisciplinary team and other managers to develop appropriate interventions to achieve clinical and functional goals.
  • Thinks and acts in a team-oriented, interdisciplinary manner.
  • Participate in the agency’s Quality Assurance Performance Improvement/PIP as assigned.
  • Competent in the knowledge of Medicare CoPs, State regulations, and JCAAHO standards
  • Implements performance improvement plan as directed.
  • Complies with organization’s specific infection control policy and procedures.
  • Participates in reporting data related to surveillance and prevention of infection.
  • Participates in infection control processes designed to reduce transmission of infection.
  • Ensures compliance with state and federal home health regulations.
  • Ensures compliance with Extended Home Care policies and procedures.
  • Demonstrates sound judgment by taking appropriate actions regarding suspected violation of corporate compliance regulations.
  • Reports all suspected violations to Supervisor, Compliance Officer or Compliance Hotline.
  • Performs other duties as assigned.

Qualifications and Experience:

  • Graduate of an approved program in physical, occupational, or speech therapy.
  • New York State License as a physical, occupational, or speech therapist.
  • Current registration from NYS Department of Education in physical, occupational, or speech therapy.
  • Minimum of one (1) year of work experience as a physical, occupational, or speech therapist in a medical setting.
  • Two years’ experience providing physical, occupational, or speech therapy in the home care setting.
  • Previous supervisory experience in home care department is preferred.
  • Demonstrates leadership and strong interpersonal communication skills.

Salary range - $115-$120K
