Santa Cruz Post Acute

Full-Time • Part-Time • PRN
Starting at $45-50/hour (DOE) + Benefits

Santa Cruz Post Acute is seeking a compassionate and motivated Physical Therapy Assistant (PTA) to join our exceptional rehabilitation team. Our skilled nursing facility is dedicated to delivering high-quality, resident-centered care in a supportive and collaborative environment. We offer full-time, part-time, and PRN opportunities with flexible scheduling.

The Physical Therapy Assistant works under the supervision of a licensed Physical Therapist to provide therapeutic treatments that enhance residents’ mobility, strength, balance, endurance, and overall functional independence.

Key Responsibilities

  • Deliver direct physical therapy treatments following individualized plans of care developed by the Physical Therapist

  • Assist residents with therapeutic exercises, gait training, mobility programs, and functional activities

  • Accurately document daily treatment sessions, resident progress, and responses to therapy in compliance with facility and regulatory standards

  • Collaborate with the Physical Therapist, therapy team, nursing staff, and interdisciplinary team members to support resident-centered goals

  • Educate residents and caregivers on exercise programs, safety strategies, and mobility techniques

  • Maintain compliance with state and federal regulations and professional standards of practice

Qualifications

  • Current Physical Therapy Assistant (PTA) license in the state of California

  • Graduate of an accredited PTA program

  • Excellent communication, teamwork, and documentation skills

  • SNF experience preferred, but new graduates are encouraged to apply
