Job Description

Westminster Baldwin Park, a new Active Living Community in Baldwin Park, is seeking a Physical Therapy Assistant (On-Call) to join our team. We are a not-for-profit, community service organization dedicated to enhancing the quality of life for older adults through compassionate care and exceptional services.

The Physical Therapy Assistant (PTA) works under the direct supervision of the Program Manager or Physical Therapist, providing care to assigned patients and residents. The level of supervision will vary based on the complexity of the patient’s condition and the nature of the skilled services required. The PTA may also receive additional clinical guidance from licensed Physical Therapists as needed.

ESSENTIAL JOB FUNCTIONS

Comply with all applicable rules, policies, standards and guidelines related to employment with Westminster Services and it's communities.

  1. Responsible for carrying out Care Plan as designed by the Physical Therapist (PT).
  2. Assist the PT during evaluations.
  3. Monitor resident’s progress and communicate with PT.
  4. Maintain documentation in system of resident’s activities, record progress and document billing activities.
  5. Perform Resident’s Home Assessment as part of the discharge process as directed by the PT.
  6. Perform Resident’s screening during the admission process and quarterly & annual screening to determine the need for therapy as directed by the PT.

Education and Experience:

  1. AA Degree from an APTA accredited program.
  2. Licensure where required by law.
  3. Two (2) years experience in a health care setting preferred.
