Key Responsibilities

• Patient Care & Rehabilitation: Provide prescribed occupational therapy services and skilled procedures to minimize physical disabilities and help patients reach their optimum level of independence.
• Assessment & Care Planning: Conduct initial assessments, develop and modify maintenance plans, perform 30-day reassessments, and collaborate with multidisciplinary home care personnel on care planning.
• Physician Collaboration: Consult directly with physicians regarding changes in patient condition or treatment plans.

Qualifications

• Passion: A genuine drive to serve others and help them thrive.
• Education: Graduate of an accredited Physical Therapist Assistant (PTA) program (CAPTE, APTA, CAHEA, or AMA approved).
• Licensure: Current Physical Therapist Assistant license in the state of practice (Required).
• Certifications: CPR Certification (Required).
• Experience: 1 year of home health experience preferred.

Position Details

• Job Type: PRN
• Pay: $38.00 – $45.00 per hour
• Schedule: Day shift, Monday to Friday (8:00 AM – 5:00 PM); weekends as needed

Compensation: $38.00 - $45.00 per hour
