What You’ll Do:

• 
  Execute therapeutic care plans
  crafted by a licensed Physical Therapist to restore function and prevent decline
• 
  Provide in-home treatments
  like range-of-motion, strengthening, balance training, and gait assistance
• 
  Track progress and report changes
  using user-friendly EMR software
• 
  Teach self-care techniques
  and guide caregivers to reinforce therapy between visits
• 
  Actively collaborate
  with PTs, nurses, and other home health professionals for optimal outcomes

Qualifications:

• 
  Licensed Physical Therapy Assistant
  in Minnesota
• 
  Valid
  driver’s license
  , insured and dependable vehicle
• 
  Compassionate communicator
  and a proactive care advocate
• 
  Home care experience preferred, but
