
Responsible for carrying out the care plan as designed by the Physical Therapist (PT) and assisting the PT during evaluations. Monitor resident's progress and communicate with PT. Maintain documentation in system of resident's activities, record progress and document billing activities. Perform resident's home assessment as part of the discharge process as directed by the PT. Perform resident's screening during the admission process and quarterly & annual screening to determine the need for therapy as directed by the PT.
Requires an AA Degree from an APTA accredited program and a current State of Floria PTA license. Two years experience in a health care setting preferred.
