POSITION

TheNorth Country Home Health &Hospice Agency Physical Therapist Assistant is responsible forproviding physical therapy treatments under the supervision of a qualified Physical Therapist in accordancewith the plan of careThe Home Health Physical Therapist Assistant will utilize therapeutic exercise,modalities and activities to achieve the highest level of function possible within the home environment. Inthisposition youwillbeworkingwithinaleadershipteamthatexpectstomaintainpatientqualityoutcomesthatscoreinthetop20%ofHomeHealthAgenciesinthecountry.

REQUIREDQUALIFICATIONS

• Associate degreefromanaccreditedphysicaltherapistassistantprogram

• MustholdacurrentandvalidPhysicalTherapistAssistantpracticelicensegrantedfromthe NewHampshireOfficeofAlliedHealthProfessions

• Goodorganizationalskillswiththeabilitytomultitask

• Excellenceincustomerserviceandcommunicationskills

• CompetentinComputeruse:Windows

• BLScertificationrequired

ESSENTIALRESPONSIBILITIES:

• Performs physicaltherapyproceduresintheclient'shome,asprescribedbythe physicianandsupervisedbyaqualifiedPhysicalTherapist.

• Administersnon-complextherapeuticexercises,therapeuticmassage,traction,heat,light,cold,water,andelectricalmodalitiestoclientswithrelativitystableconditions.

• Instructs,motivates,andassistsclientsinlearningandimprovingfunctionalactivities.

• Instructspatientsandfamilymembersontheproperuseofprostheticdevices.

• Observesclient'sprogressandresponsetotreatmentandreportsfindingstoPhysicalTherapist.

• ModifiesPlanofTreatment,asauthorizedbythephysician, aftersubsequentauthorization,instruction,andsupervisionbythePhysicalTherapist.
• Responsibleformaint,1-hingcurrentprogressnotesinthepatientsrecordPatientprogressinrelationto goals to be noted, typeoftreatment rendered and evidence 01 1-1eriodic reviews of the referringprovideralsotobenotedinrecord.
• Ensuresthatalldocumentationislegibleand, in the format,approvedbythedepartment:SOAPnotesfordailyvisitsandanarrativeformatforaninitialevaluationispreferred.
• Ensuresthatequipmentusedbythepatient isinsafeworkingconditionpriortousebythepatientandthatanyunsafeequipmentwillbereportedtotheClinicalNurseManager
• AttendsmeetingsasdesignatedbytheClinicalNurseManager.
• WorkswiththeClinicalNurseManagerandsupervisingPhysicalTherapisttodevelopnewprogramsandcollaborateonspecificdepartmentalprojectsbasedonpatient needs
• Demonstratesknowledgeandskillsnecessarytoprovideappropriatecarewithinthepatient'shome
• Demonstrates knowledge of appropriate modification of treatment programs and safety principles forprovidingcarewithinthehome.
• Managestimeeffectivelytoensurethatdeadlinesaremetandthatpersonalprioritiessupportorganizationalobjectives
• Performsallotherdutiesasassigned.
