This is a fully on-site Physical Therapist position offering the opportunity to provide meaningful, patient-centered care in a collaborative outpatient environment. Candidates must hold active New York Medicare and Medicaid provider IDs to be considered for this role.

Position Overview

US Career Partners is recruiting on behalf of a well-established outpatient medical center in Brooklyn, NY, seeking a compassionate, skilled, and dedicated Physical Therapist to join their team. The ideal candidate brings strong clinical knowledge, excellent communication skills, and a genuine commitment to helping patients achieve their rehabilitation goals. This role is well-suited for a PT who enjoys working directly with patients in an outpatient setting and values being part of a collaborative healthcare environment.

Schedule

Sunday, Tuesday & Thursday: 9:00 AM – 5:00 PM

Monday & Wednesday: 11:00 AM – 7:00 PM

Responsibilities

• Provide comprehensive physical therapy evaluations and treatment to patients in an outpatient setting
• Develop individualized treatment plans based on patient needs, diagnoses, and functional goals
• Perform therapeutic exercises, manual therapy, and other appropriate physical therapy interventions
• Monitor patient progress and adjust treatment plans as needed
• Educate patients and caregivers on exercises, mobility, injury prevention, and home treatment plans
• Maintain accurate and timely clinical documentation
• Collaborate with physicians and other healthcare professionals to ensure coordinated patient care
• Help patients improve strength, mobility, function, and overall quality of life
• Follow established clinical, regulatory, and documentation standards

Qualifications

• Doctor of Physical Therapy (DPT) or equivalent accredited Physical Therapy degree
• Active New York Physical Therapist license
• Active New York Medicare and Medicaid provider IDs — required
• Strong clinical and assessment skills
• Excellent communication and interpersonal skills
• Compassionate, patient-centered approach
• Ability to work effectively both independently and as part of a multidisciplinary team
• Strong organizational and documentation skills
• Commitment to providing high-quality care to a diverse patient population
