A home health provider in the Ventura, CA area is seeking a contract Physical Therapist fluent in both English and Spanish. This per diem role offers flexibility and autonomy, ideal for clinicians who enjoy delivering one-on-one care in a home setting.
Responsibilities:
Conduct in-home patient evaluations and develop individualized treatment plans.
Deliver therapy services based on physician orders and patient needs.
Educate patients and caregivers to support continued progress between visits.
Document all visits, outcomes, and communications accurately and in compliance with state and agency standards.
Coordinate with interdisciplinary team members to ensure comprehensive patient care.
Qualifications:
Active California Physical Therapist license.
Bilingual in English and Spanish (required).
Minimum of 1 year of PT experience; home health experience preferred.
Comfortable working independently in the field.
Familiarity with home health documentation platforms is a plus.
Compensation:
Competitive per visit or hourly rates available— typically $55 to $70 per visit, depending on experience.
