Sangre de Cristo Community Care is seeking a compassionate and motivated Full-Time Physical Therapist to join our growing Home Health team. This position is based in Pueblo, Colorado, providing care throughout our service area, including travel to La Junta as needed.

About the Role

As a Home Health Physical Therapist, you'll have the opportunity to build relationships with patients and families while developing individualized treatment plans that make a meaningful difference in their everyday lives.

You'll:

• Perform comprehensive physical therapy evaluations in patients' homes
• Develop and implement individualized, evidence-based plans of care
• Help patients improve mobility, strength, balance, safety, and independence
• Provide skilled therapeutic interventions based on each patient's goals and needs
• Educate patients and caregivers on exercises, mobility techniques, equipment, fall prevention, and home safety
• Monitor patient progress and adjust treatment plans as appropriate
• Collaborate with physicians, nurses, therapists, and other members of the interdisciplinary care team
• Maintain timely and accurate clinical documentation
• Help patients achieve their highest possible level of function and quality of life

Compensation & Benefits Include:

• Competitive Salary: $94,000–$112,000 annually
• Mileage Reimbursement: $0.625 per mile

Requirements

• Must be at least 21 years of age
• Graduate of an accredited Physical Therapy program (APTA/CAHEP) with a B.S., B.A., or M.S. degree
• Licensed or eligible for licensure as a Physical Therapist in the State of Colorado
• Valid, non-restricted driver’s license and reliable transportation
• Ability to work a weekend schedule as needed
• Minimum of two (2) years of experience required; home health experience preferred
