Physical Therapist (PT) – PRN Weekends

Location: Sunnyvale Post Acute – Sunnyvale, CA

Pay Rate: $57–$67 per hour (DOE)

Schedule: PRN / Weekends

We are seeking a talented PRN Weekend Physical Therapist to join our rehab department.

Position Summary

The PRN Physical Therapist evaluates, plans, and provides patient‑centered physical therapy services for individuals requiring rehabilitation in a post‑acute/long‑term care setting. You will collaborate closely with an interdisciplinary team to ensure the best rehabilitation outcomes for our residents.

Responsibilities

  • Complete PT evaluations and develop individualized treatment plans
  • Provide effective, evidence‑based therapeutic interventions
  • Document treatments, progress notes, and evaluations in accordance with facility and regulatory requirements
  • Communicate patient progress with interdisciplinary team members
  • Educate patients, families, and caregivers on therapy goals, safety, and home programs
  • Ensure safe, ethical, and high‑quality care at all times

Qualifications

  • Current California Physical Therapist (PT) license
  • Graduate of an accredited PT program (DPT or MSPT)
  • Previous SNF or post‑acute rehab experience preferred (not required)
  • Strong clinical reasoning and communication skills
  • Ability to work independently and collaboratively
