San Francisco Post Acute is a premier post-acute rehabilitation and skilled nursing facility dedicated to helping patients recover, rebuild, and transition back to their highest level of independence. We pride ourselves on a supportive, patient-centered environment, and we are looking for a skilled and compassionate Physical Therapist (PT) to join our dynamic therapy team on a PRN basis.

Position Overview

We are seeking a PRN Physical Therapist who is passionate about delivering exceptional geriatric and post-acute care. Key Responsibilities

• Evaluate patients, develop individualized treatment plans, and establish realistic, functional goals.
• Administer evidence-based physical therapy interventions to improve mobility, strength, balance, and coordination.
• Document patient progress, updates, and discharge plans accurately and timely in compliance with facility and regulatory standards.
• Collaborate closely with a multidisciplinary team, including physicians, occupational therapists, speech-language pathologists, and nursing staff.
• Educate patients, families, and caregivers on home exercise programs and safe mobility techniques.

Qualifications

• Minimum of a Bachelor’s, Master’s, or Doctorate degree in Physical Therapy from an accredited program.
• Active, unrestricted Physical Therapist license in the State of California.
• Current BLS/CPR certification.
• Previous experience in a skilled nursing facility (SNF), acute care, or post-acute rehab setting is preferred, but open to all qualified applicants.
• Strong communication, organizational, and interpersonal skills.
