Pleasant Hill Post Acute is seeking a compassionate and skilled Physical Therapist (PT) to join our in-house rehabilitation team. Responsibilities

  • Evaluate residents and develop individualized treatment plans

  • Provide skilled physical therapy services to improve mobility, strength, and independence

  • Supervise PTAs and collaborate with the interdisciplinary care team

  • Participate in care planning and discharge preparation

  • Educate residents, families, and caregivers

  • Maintain accurate and timely clinical documentation

Qualifications

  • Current California Physical Therapist (PT) license

  • Strong clinical and communication skills

  • Commitment to resident-centered care

  • Ability to work effectively within a multidisciplinary team
