Position Details
Position: Physical Therapist (PT) – PRN
Location: Concord, CA
Compensation: Starting at $64–$65/hour
Schedule: PRN / Per-Diem
Environment: Supportive, team-oriented, and fast-paced rehab setting
Key Responsibilities
Perform comprehensive physical therapy evaluations in a timely manner following physician referral
Develop and implement individualized treatment plans focused on mobility, balance, strength, pain management, and functional independence
Supervise and collaborate with Physical Therapist Assistants (PTAs) to ensure quality patient care and compliance with state practice standards
Participate in interdisciplinary care planning, patient conferences, and utilization review meetings
Recommend appropriate Durable Medical Equipment (DME) to support safe mobility and discharge planning
Educate residents, families, and caregivers on exercise programs, safety strategies, and long-term maintenance plans
Maintain accurate and timely documentation in accordance with regulatory and payer guidelines
Qualifications
Current California Physical Therapist (PT) license in good standing
