Position Responsibilities
As a Physical Therapist, you will help residents improve their strength, mobility, function, and independence through individualized, evidence-based care.
Responsibilities include:
Perform comprehensive physical therapy evaluations and develop individualized treatment plans.
Provide skilled physical therapy services in accordance with the California State Practice Act.
Monitor resident progress and modify treatment plans as clinically appropriate.
Supervise Physical Therapist Assistants (PTAs) and collaborate with Restorative Nursing staff to promote continuity of care.
Complete accurate and timely clinical documentation, including treatment notes, progress reports, and recertifications.
Collaborate with nursing and the interdisciplinary care team to support comprehensive resident care.
Participate in care planning meetings and utilization reviews as needed.
Educate residents and families regarding mobility, safety, home exercise programs, and discharge planning.
Assess and coordinate appropriate durable medical equipment (DME), including walkers, wheelchairs, braces, and other mobility equipment.
Promote safe mobility and functional independence throughout the rehabilitation process.
Qualifications
Current and unrestricted California Physical Therapist (PT) license required.
