About Us

Chandler Creek Post Acute is a 133-bed skilled nursing facility located in Greer, South Carolina. We are known for our small-town feel, strong community connections, and commitment to providing compassionate, high-quality care.

We are seeking a dedicated Physical Therapist (PT) to join our full-time rehab team. If you’re passionate about helping patients regain independence and improve quality of life, we’d love to have you on our team.

As a Physical Therapist (PT), you will evaluate, develop, and implement individualized treatment plans to help residents improve mobility, reduce pain, and achieve their highest level of independence.

Key Responsibilities

  • Conduct patient evaluations and develop customized treatment plans
  • Provide therapeutic interventions to improve strength, mobility, and function
  • Monitor patient progress and adjust treatment plans as needed
  • Educate residents and families on therapy techniques and goals
  • Maintain accurate and timely clinical documentation
  • Collaborate with interdisciplinary teams to ensure optimal patient outcomes

Qualifications

  • Bachelor’s degree required; advanced degree preferred
  • Active, unencumbered Physical Therapist license in South Carolina
  • Experience in a long-term care or rehab setting preferred
  • Familiarity with PointClickCare and Casamba preferred
  • Strong communication and clinical decision-making skills
  • Passion for patient-centered care
