Physical Therapist (PT) - Home Health

$10,000 Sign-On Bonus!

Redcrest Home Healthcare & Hospice is seeking a compassionate and dedicated Physical Therapist (PT) to join our growing Home Health team. If you're passionate about helping patients regain independence while enjoying autonomy and a Monday-Friday schedule, we'd love to hear from you!

Territory: Hamilton, Boone, Hendricks, Marion, and Johnson Counties
Schedule: Full-Time | Monday-Friday | 8:15 AM-5:00 PM

As a Physical Therapist (PT), you'll provide skilled home health therapy services under the direction of the attending physician. The PT evaluates patients, develops individualized treatment plans, provides patient and caregiver education, and helps patients achieve their highest level of function while maintaining compliance with Medicare, state, and agency regulations.

What You'll Do

  • Provide skilled physical therapy services in patients' homes.
  • Complete evaluations, OASIS assessments, reassessments, recertifications, resumptions of care, and discharge visits.
  • Develop, implement, and modify individualized plans of care.
  • Help patients improve mobility, reduce pain, restore function, and maximize independence.
  • Educate patients and caregivers on diagnoses, treatment plans, exercises, safety, fall prevention, and proper use of assistive devices.
  • Collaborate with physicians and the interdisciplinary care team to ensure quality patient outcomes.
  • Supervise and communicate with Physical Therapist Assistants (PTAs) as appropriate.
  • Promote continuity of care through effective admissions, transfers, and discharge planning.
  • Complete accurate clinical documentation within required company and Medicare timeframes.
  • Participate in case conferences, on-call rotation, required trainings, and agency meetings.
  • Travel throughout the assigned territory to provide patient care.

Qualifications

  • Current Physical Therapist (PT) license in the state of employment.
  • Minimum of 1 year of PT experience in an acute care setting required.
  • Home Health experience preferred.
  • Strong clinical judgment and problem-solving skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently while managing daily schedules.
  • Valid driver's license, reliable transportation, and current auto insurance.

Physical Requirements

This position requires frequent standing, walking, bending, reaching, lifting up to 50 pounds (with or without assistance), driving between patient homes, and completing clinical documentation. Reasonable accommodations may be made to enable qualified individuals to perform the essential functions of the position.
