Pacific Coast Post-Acute is seeking a dedicated and compassionate Physical Therapist (PT) to join our exceptional in-house rehabilitation team in Salinas, California.

Key Responsibilities

  • Perform comprehensive physical therapy evaluations and develop individualized treatment plans based on resident goals and functional needs.

  • Provide skilled physical therapy interventions, including gait training, therapeutic exercise, balance training, manual therapy, and neuromuscular re-education.

  • Monitor resident progress and adjust treatment plans to maximize functional outcomes.

  • Guide residents through their rehabilitation journey from acute recovery toward safe discharge and community reintegration.

  • Collaborate with interdisciplinary team members to coordinate effective, resident-centered care.

  • Supervise Physical Therapist Assistants (PTAs) in accordance with California practice guidelines.

  • Maintain accurate and timely documentation, including evaluations, daily treatment notes, progress reports, recertifications, and discharge summaries.

  • Participate in care conferences, clinical meetings, and quality improvement initiatives.

Qualifications

  • Current California Physical Therapist (PT) license in good standing.

  • Strong clinical reasoning, communication, and teamwork skills.
