What You'll Do

As a Physical Therapist, you'll evaluate, develop, and implement individualized treatment plans that help residents improve mobility, restore function, and maximize independence.

Your responsibilities will include:

  • Perform comprehensive physical therapy evaluations

  • Develop and implement individualized treatment plans based on resident goals

  • Provide skilled therapeutic interventions to improve strength, balance, mobility, and overall function

  • Collaborate with physicians, nursing staff, and the interdisciplinary care team to coordinate resident care

  • Educate residents and families on therapeutic exercises, mobility techniques, and fall prevention

  • Accurately document evaluations, treatments, progress notes, and discharge planning

  • Monitor resident progress and adjust treatment plans to maximize outcomes

Qualifications

  • Current California Physical Therapist (PT) license (or eligibility for licensure)

  • Strong clinical, communication, and problem-solving skills

  • Passion for rehabilitation and resident-centered care

  • Positive, collaborative, and compassionate approach to patient care
