Peninsula Post Acute is a premier short-term rehabilitation and skilled nursing facility dedicated to helping residents recover, regain independence, and return to their highest level of function. We are seeking a compassionate and motivated Physical Therapist (PT) to join our collaborative in-house rehabilitation team on a PRN basis.

Key Responsibilities
Evaluate residents and develop individualized physical therapy treatment plans.
Provide evidence-based physical therapy services to improve mobility, strength, balance, endurance, and overall functional independence.
Monitor resident progress and modify treatment plans as appropriate to achieve optimal outcomes.
Maintain accurate and timely documentation, including evaluations, daily treatment notes, progress reports, recertifications, and discharge summaries.
Collaborate with physicians, nursing staff, rehabilitation professionals, residents, and families to coordinate high-quality patient care.
Supervise Physical Therapist Assistants (PTAs) in accordance with California practice guidelines.
Participate in interdisciplinary care conferences and discharge planning.
Educate residents and caregivers on home exercise programs, fall prevention strategies, and safe mobility techniques.
Qualifications
Current California Physical Therapist (PT) license in good standing.
Strong communication, clinical reasoning, and teamwork skills.
Passion for delivering high-quality rehabilitation services.
