- Provide skilled physical therapy services to patients in a skilled nursing facility, including long term and short term rehab.
- Collaborate with the therapy team to develop and implement individualized treatment plans.
- Adapt therapy approaches to meet the diverse needs of patients across various stages of recovery.
- Document patient progress and communicate effectively with team members and healthcare providers.
Skills and Qualifications
- Valid state licensure as a Physical Therapist or Physical Therapist Assistant.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Flexibility to work on an as-needed basis, aligning availability with staffing needs.

Requirements
Applicants must have an active Kentucky Physical Therapist or Physical Therapist Assistant license or must have the necessary qualifications to earn Kentucky license. These qualifications include earning an advanced degree from a Commission on Accreditation in Physical Therapy Education-accredited physical therapist education program, passing the state licensure exam, and fulfilling the state’s requirements for licensure.
