Job Description

Systematically evaluates the strengths, needs, preferences, and challenges of infants and children, in collaboration with the family, as part of a NJ Early Intervention Team. Collaborates with family and other Individual Family Service Plan Team Members to develop an Individualized Family Service Plan (IFSP) which incorporates family priorities/concerns, clinical observation, and evaluation/assessment data to develop child and family outcomes. Implements evidenced-based instructional practices in the child's natural environment. Provides education, resources, and coaching to clients, family, caregivers, and health care professionals to promote and maintain health and well-being.

QUALIFICATIONS: Bachelor of Science Degree in Physical Therapy required. 1 year of documented professional experience with individuals from birth to five years of age who have developmental delays and disabilities. Licensed Physical Therapist, State of New Jersey.

Position Responsibilities

Provides necessary clinical services:

  • Practices in accordance with professional code of ethics and state practice laws.
  • Recognizes client focused techniques for both internal and external clients.
  • Continually strives to address patient concerns proactively so that experiences associated with the Child Development Center and Early Intervention Program services are positive.
  • Meets all unit specific standards for planning.
  • Participates in individual family services as assigned.
  • Revises plan of care in accordance with regulatory agencies.
  • Participates in discharge planning.
  • Assists in facilitating a smooth transition from the NJ Early Intervention System.
  • Implements the IFSP treatment plan by utilizing appropriate, evidenced based hands-on treatment and instructional practices, family/caregiver coaching, equipment and support personnel to achieve desired results.
  • Demonstrates and instructs family and caregivers in skills necessary to improve the child's level of function and in the use of equipment and home programs.
  • Conducts valid evaluations/assessments within appropriate timeframes.
  • Evaluates child by reviewing development, birth and medical history, hands on examination, clinical observation, and parental report.
  • Recognizes developmental variations.
  • Interprets results of child's evaluation to identify strengths and challenges to develop an IFSP in collaboration with parents/guardians and NJEIS Service Coordinator.
  • Performs ongoing assessment, reviewing all areas of development, and completes "Practitioner Review Summary" for Periodic and Annual Reviews.
  • Collaborates with family and IFSP Team and makes recommendations to modify the IFSP, as appropriate.
  • Serves as a consultant as needed.
  • Provides physical therapy services to children birth to three years.
  • Provides prevention, screening, consultation, assessment, treatment, management and follow-up services for:
    • Orthopedic anomalies and/or concerns of the spine, trunk, and lower extremities.
    • Analysis of posture, gait patterns, locomotion and balance.
    • Analysis of appropriate adaptive and mobility devices.
    • Addresses positioning and strategies as related to feeding, play skills, ADL and augmentative communication.
    • Gross motor delays.
    • Tonal abnormalities. • Motor-planning and sensory awareness issues.
    • Collaborates with orthotist and prosthetist in selection and management of orthotic/prosthetic.
    • Assists families' understanding of specific diagnosis and impact on gross motor development.

Maintains Clinical Records:

  • Completes all IFSP service logging, Evaluation Reports, Practitioner Reviews, and Assessment Reports in the Early Intervention Management System within 24 hours of service delivery.
  • Maintains and submits accurate records for quality monitoring and legal purposes.
  • Accurately Completes NJEIS generated service verification logs and has parent/caregiver sign log at the end of each NJEIS service visit.
  • Documents all abuse completely, appropriately, and in a timely manner consistent with state regulation and Inspira policy.
