Job Description

Tap Healthcare Solutions is seeking a dedicated and compassionate Physical Therapist – Outpatient Neuro to join the rehabilitation team at Providence in Medford, Oregon In this role, you will provide specialized outpatient neurological rehabilitation services to patients recovering from neurological injuries, illnesses, and conditions while helping them improve mobility, independence, and overall quality of life.

You will collaborate closely with physicians, rehabilitation specialists, and interdisciplinary care teams to develop individualized treatment plans focused on patient-centered outcomes. This position offers competitive compensation ranging from approximately $96,000 to $149,000 annually based on experience, along with a sign-on bonus for eligible candidates and the opportunity to work within a highly collaborative rehabilitation environment.

Required Qualifications

• 
  Graduate of an accredited Physical Therapy program with one of the following:
  • Bachelor's degree in Physical Therapy, OR
  • Master's degree in Physical Therapy, OR
  • Doctor of Physical Therapy (DPT) degree.
• Active Oregon Physical Therapist license or Washington Physical Therapist license upon hire.
• 
  Ability to obtain and maintain licensure in states where care is delivered as required.
• Basic Life Support (BLS) certification through the American Heart Association upon hire.
• 
  Willingness to travel as needed to support patient care delivery.
• 
  Specialized physical therapy training or experience may be required depending on departmental needs.

  Experience in neurological rehabilitation and outpatient therapy settings

  Strong communication and patient engagement skills.
• 
  Ability to work effectively within a multidisciplinary rehabilitation team
