Experienced Physical Therapist- Outpatient clinic & Home Care Settings

FlexStaff is seeking a dedicated Physical Therapist on behalf of our client to deliver compassionate, innovative care to individuals with acute or chronic disabilities across our healthcare centers, day programs, and residential settings.

Be at the forefront of rehabilitation by designing and implementing personalized treatment programs that restore function and foster independence. Work closely with a multidisciplinary team, supervise talented PTAs and aides, and see the tangible impact of your efforts every day!

Responsibilities:

- **Develop & Implement Treatment Plans:** Assess individuals’ needs and craft tailored therapy programs utilizing modalities such as hydrotherapy, ultrasound, electrical stimulation, and therapeutic exercises.

- **Evaluate & Monitor Progress:** Conduct annual screens, assessments, and daily documentation to track improvements and adjust treatments accordingly.

- **Collaborate & Communicate:** Work with physicians, caregivers, and families to ensure the best outcomes. Participate in care planning and team meetings.

- **Supervise & Lead:** Oversee PTAs and aides, providing guidance and oversight in healthcare, day programs, and residential environments.

- **Ensure Compliance & Quality:** Maintain accurate documentation within ECW, adhere to NYS and federal regulations, and contribute to quality improvement initiatives.

- **Educate & Empower:** Provide training to individuals, families, and staff to promote sustained progress and independence.

- **Travel & Oversee:** Oversee therapy services across multiple sites, ensuring consistency and excellence in care.

Qualifications:

- Graduate of an accredited physical therapy program with a Bachelor's degree or higher

- Licensed, eligible, or limited permit holder in NY State

- Skilled in PT evaluation, treatment modalities, and therapeutic exercises

- Excellent communicator with proficient Microsoft Office skills

- Physically capable of performing transfers and utilizing equipment as needed

- Willing to travel between assigned locations and oversee staff across various settings

- Committed to ongoing learning and compliance with all regulations
