As a home health physical therapist, you’ll be part of a collaborative interdisciplinary team including RNs, LPNs, LNAs, Occupational Therapists, Speech-Language Pathologists, Social Workers, and administrative staff 🤝, providing comprehensive, patient-centered care.

As an Adult Home Care Physical Therapist, you will:
Perform comprehensive evaluations and assessments 🩺
Develop individualized treatment plans tailored to each patient’s needs ✍️
Deliver skilled therapy interventions to improve mobility, strength, and function 💪
Support patients in remaining in their homes safely and independently 🏡
Our coverage area is segmented into four territories within Chittenden and Grand Isle Counties Case load territory is assigned after orientation.
Bachelor’s degree or higher in Physical Therapy
2+ years of general PT experience preferred, or 1 year at PT I level within UVM Health Home Health & Hospice with strong performance
Home health experience preferred, but not required. We will mentor and support you.
Current Vermont Physical Therapist license, or willingness to obtain one 🌱
