We are looking for therapists that are available weekdays.

As a home health physical therapist, you’ll be part of a collaborative interdisciplinary team including RNs, LPNs, LNAs, Occupational Therapists, Speech-Language Pathologists, Social Workers, and administrative staff 🤝, providing comprehensive, patient-centered care.

About the Role

As an Adult Home Care Physical Therapist, you will:

  • Perform comprehensive evaluations and assessments 🩺

  • Develop individualized treatment plans tailored to each patient’s needs ✍️

  • Deliver skilled therapy interventions to improve mobility, strength, and function

  • Support patients in remaining in their homes safely and independently

Our coverage area is segmented into four territories within Chittenden and Grand Isle Counties Case load territory is assigned after orientation.

Minimum Qualifications

  • Bachelor’s degree or higher in Physical Therapy

  • 2+ years of general PT experience preferred, or 1 year at PT I level within UVM Health Home Health & Hospice with strong performance

  • Home health experience preferred, but not required, we can train the right person!

  • Current Vermont Physical Therapist license, or willingness to obtain one
