Responsibilities
Conduct comprehensive patient evaluations in the home setting
Develop and implement individualized treatment plans
Provide therapeutic interventions to improve strength, mobility, balance, and safety
Educate patients and caregivers on home exercise programs and fall prevention
Coordinate care with physicians and interdisciplinary team members
Complete accurate and timely documentation
Ensure compliance with Medicare and home health regulations
Qualifications
Active Physical Therapist (PT) license
Home Health experience preferred but not required
Strong clinical assessment skills
Valid driver’s license and reliable transportation
Ability to work independently in the field
Excellent communication and organizational skills
