Job Description

Key Physical Therapist Responsibilities

• Conduct comprehensive assessments to develop individualized therapy plans for patients.
• Implement therapeutic interventions to enhance patient mobility, strength, and functionality.
• Educate patients and families on therapy goals and techniques for home-based care.
• Collaborate closely with a multidisciplinary team to ensure cohesive patient care.
• Maintain accurate documentation of patient progress and modify therapy plans as needed.
• Ensure patient safety and adhere to all health regulations and company policies.

Work Schedule and Travel Area

● Schedule (Including any on call/weekends): M-F 8am-5pm; full time; 1 weekend/month; 2 holidays/year; no on call. This position supports patients in Montoursville and surrounding areas.

Essential Qualifications

• Valid PT license in the state of practice with Bachelor's Degree
• Minimum of 2 years of experience in physical therapy, with preference for home health experience.
• 1+ years of experience in EMR, Other EMR and Practice Partner EMR
• Valid DL
• Strong understanding of home health therapy protocols and patient education.
• Excellent communication, organizational, and documentation skills.
• Ability to work independently and adapt to various patient needs.
