What You'll Do


  • Evaluate patients in their homes and establish individualized treatment plans.

  • Improve mobility, balance, strength, endurance, and functional independence.

  • Recommend adaptive equipment and home safety modifications when appropriate.

  • Educate patients and caregivers to maximize long-term outcomes.

  • Collaborate with physicians, nurses, occupational therapists, speech therapists, and the interdisciplinary care team.

  • Maintain timely and accurate clinical documentation.


What We're Looking For


  • Active Minnesota
    Physical Therapist (PT)

    license.

  • Strong clinical reasoning and patient-centered approach.

  • Excellent communication and relationship-building skills.

  • Ability to work independently while managing a flexible field schedule.

  • Valid driver's license and reliable transportation.
