About the Role:
What You’ll Do:
Provide one-on-one outpatient physical therapy to patients with a mix of neurological and orthopedic diagnoses.
Evaluate and treat patients with conditions such as stroke, TBI, Parkinson’s disease, balance/vestibular disorders, postoperative orthopedic cases, sports injuries, spine dysfunction, and chronic pain.
Develop individualized plans of care based on patient goals, functional needs, and clinical findings.
Deliver skilled interventions including therapeutic exercise, neuromuscular re‑education, gait training, manual therapy, balance retraining, and task-specific motor learning.
Educate patients and caregivers on home programs, safety, and long-term wellness strategies.
Communicate and collaborate with referring providers, interdisciplinary team members, and families.
Maintain accurate and timely documentation to support high-quality continuity of care.
Collaborate with referring physicians and fellow rehab professionals.
