Job Description

Are you a Physical Therapist with a passion for helping medically complex children reach their highest potential? Home of the Innocents is seeking a Physical Therapist to provide specialized rehabilitation and habilitation services at the Kosair for Kids Complex Care Center our pediatric skilled nursing facility. In this rewarding role, you’ll deliver individualized therapy to residents with complex medical needs, including tracheostomies, ventilator dependency, and enteral feedings, while collaborating with an expert interdisciplinary team dedicated to enriching every life they touch.

A Day in the Life
The Physical Therapist assesses, evaluates, develops, and implements individualized plans of care to support mobility, gross motor development, range of motion, and functional independence. Reporting to the Lead Physical Therapist, you’ll collaborate with PTAs, Occupational Therapy, Speech Therapy, Nursing, and the broader interdisciplinary team to promote the highest practicable quality of life, meeting or exceeding federal, state, local, and facility standards.

Schedule: Full-time, 30 hours per week. Flexible schedule required, must be able to meet the needs of residents (early morning &/ or later afternoons). Must be able to work Fridays for coverage in the department. May include therapy outdoors, pool, offsite, and in the community.
How This Position Contributes to Our Mission
• Deliver evidence-based PT services including gross motor skill development, range of motion, mobility, positioning, transfers, aquatic therapy, and orthotics/splinting per physician orders and individualized care plans
• Conduct comprehensive evaluations at admission, significant change in condition, return from hospitalization, and annually; develop individualized care plans for medically complex residents
• Maintain accurate, timely documentation in the EMR including progress notes, care plan contributions, MDS coding, recertifications, and discharge summaries
• Clinically supervise PTAs in accordance with licensure requirements, mentoring them to achieve resident-centered goals
• Collaborate with the interdisciplinary team and communicate clearly with legal guardians regarding resident progress, functional changes, and treatment plans
• Educate caregivers, staff, and students on PT interventions, mobility equipment, and safety procedures; participate in care plan meetings and new employee orientation

Skills and Qualifications
Required:
• Doctoral degree in Physical Therapy, or 10 years of experience in the field of Physical Therapy
• Unencumbered Physical Therapist (PT) license in Kentucky
• Current CPR/BLS certification (provided if needed)
Preferred:
• Experience with pediatric or young adult populations with developmental, intellectual, or complex medical needs
• Experience in long-term care or complex care settings
• Specialty experience in orthopedics, wheelchair/positioning, orthotics, gait training, or aquatic therapy
